In the swamp lands of Louisiana the dead are buried in tombs above ground rather than interred in the waterlogged earth. Observation of this reversal of the usual arrangements prompted TP Werblin to conceive the procedure of epikeratoyhakia in 1979.' In this keratorefractive technique, instead of attempting direct invasive modification of the corneal contour, a preshaped refractive lamella is simply placed on top of the existing corneal structure (fig 8.1).
